The concept of “Fixed-on-2” and “Step-by- Step Dynamics” in edentulous patient: provocation or clinical reality?


The present socio-economic context and the need to reduce costs and treatment times result in the request for less expensive therapies in biological and economic terms, and thus the simplification of implant prosthetic protocols can be a treatment opportunity in the modern context. The reduction of the number of implants and the simplification of the prosthetic construction can have a positive impact on the overall rehabilitation cost. From the patient’s viewpoint, the rehabilitation consisting in immediate load with minimum surgical trauma, possibly with a flapless procedure, is certainly an advantage. Following some clinical works with a high scientific impact (RCT) carried out in our center, we might think it possible to predict an approach for the rehabilitation of the edentulous mandible with an immediate loading protocol through the insertion of only 2 (FO2/Fixed-on-2) or 3 implants (FO3/Fixed-on-3). In the past 7 years, 315 patients have been treated with success levels comparable to those of the classic “All-on-4” treatment. Moreover, the success of these methods also in the maxilla could open up new scenarios in the rehabilitation of fully edentulous patients: the focus would be on the clinical case, and the ultimate goal would be the best ad hoc result, with the best simplification of methods and techniques without reducing effectiveness. At least 10 years of follow-up will certainly be necessary to adequately evaluate this new therapeutic concept. After an accurate planning and a careful examination of the prosthetic components to be utilized, we have tried to reach a new outlook on implant prosthetic rehabilitations. In brief, the “LOW COST-HIGH QUALITY” concept could be the most modern paradigm to be adopted in implants prosthetics.
