Diabetes and Oral Health


Background & Aim

Diabetes mellitus (DM) is one of the most common chronic metabolic disorders. Our research aimed to demonstrate the importance of strategic presurgical considerations of metabolic levels to create a pathway in the Semmelweis University (SU), Budapest, Hungary. The model is adapted to Medical University Graz (MUG), Austria as well. Oral screening and preventive examination are crucial for people living with DM.

Methods

A retrospective study in the Oral and Maxillofacial Department of SU was performed in oral tumor patients. Diabetes-Dental Ambulances were organized for type-1 and type-2 DM patients as well. Patients under 18 and living with DM were also examined. The so-called “Budapest-pilot” provides a point-of-care examination with HbA1C, blood glucose (BG), keton measuring, and oral screening to monitor the metabolic status prior to surgical or dental examination. International guidelines were considered. All newly diagnosed type-1 and type-2 DM patients were offered to participate in this study referred from the Internal Medicine Department of the Semmelweis University.

Results

3 Departments from the Faculty of Dentistry and two departments from the Faculty of Medicine were involved in our pilot study. Hungarian Diabetes Association and International Diabetes Federation supported the initiative. Weekly office hours were organized for patients with DM. We detected elevated BG and HbA1C levels significantly higher prevalence among oral tumor patients than the control group. The pilot is under organization in MUG.

Conclusions

It may be necessary to continuously monitor the patient’s blood sugar level to maintain euglycemic levels when managing diabetes mellitus. We are willing to help create an international consensus on metabolic screening before implant or periodontological surgeries to eliminate the risk factor of our successful treatment.
